Meet the Team

Antonio Booker
I am proud native of the West End of Louisville, Kentucky. My journey into healthcare began early, shaped by a strong sense of community and a passion for science. After graduating from Louisville Male High School, I pursued a Bachelor’s degree in Biology with a concentration in Chemistry from Thomas More College (now University), completing my undergraduate studies in 2013.
It was during this pivotal transition from high school to college that my path to pharmacy began to take shape, sparked by a chance encounter at a local gas station in 2009 with Charles Taylor, who later became my mentor. That meeting led to my first job in a pharmacy and solidified my interest in the field.
I went on to earn my Doctor of Pharmacy (PharmD) from the University of Kentucky, where I was inspired by forward-thinking mentors and a curriculum that broadened my understanding of the vital role pharmacists play in the healthcare system.
My professional career began in community pharmacy, where I served as an intern at Rite Aid and was promoted to Pharmacy Manager/PIC just six months after graduating. Following the merger with Walgreens and a subsequent transition to Walmart, I sought a more meaningful connection to my work and found it in Home Infusion. There, I specialized in Infectious Disease and Pediatric Oncology, discovering a deep sense of fulfillment inpatient-centered care.
In a full-circle moment, I returned to my roots as Pharmacy Director at Park Duvalle Community Health Center in Louisville’s West End. Under my leadership, the pharmacy experienced a transformative turnaround. My team provided thousands of vaccinations, expanded clinical services, and launched a pharmacist-led HIV prevention clinic, further strengthening our impact on the community.
My next step I served as the Director of Pharmacy and Medical Operations at Isaiah House Treatment Centers, where we built a pharmacy program from the ground up. I also played a key role in the organization’s transformation into a Federally Qualified Health Center (FQHC), expanding access to care for underserved populations. In this dual role, I oversaw both pharmacy and medical operations, united by a singular goal: delivering comprehensive, compassionate healthcare to individuals battling Addiction Use Disorder and other complex medical needs.
Currently I serve as the Chief Clinical Officer for Hometown Health System and Hometown Connect. This venture is to bring affordable, comprehensive, and compassionate healthcare to one of Louisville’s most forgotten about and marginalized communities. The implementation of a primary care clinic, STI/HIV/HepC clinic, and in house pharmacy will give more access to healthcare for those in need.
Driven by purpose and grounded in service, I continue to strive to be a selfless leader, patient advocate, a child of God, and change agent in both pharmacy and public health.
